Just a tip: to turn of the Rockstar cloud (which for me doesn't work right anyway; the game at close doesn't sync but simply complains that there's a conflict if you in fact saved) you don't need to fully reinstall; closing down the game and deleting/renaming your user's Documents\Rockstar Games\GTA IV\Profiles\\cloudsavedata.dat should have the RGL re-ask on restarting the game.

This article deals with the format of a save game file for the PC version of GTA 4. That's 20 for versions before Complete Edition, and 23 for Complete Edition.
This looks like a very new problem linked to the Complete Edition of the game, where it can no longer identify save games in the profile folder. Every single time I try to open the load menu, the game flashes a list of "ghost" slots (explained in the next paragraph) and then returns a "no saved games present on the hard disk drive" message.
